Kanban

At RHLM, Kanban is implemented as a visual workflow system to improve operational clarity, control work-in-progress, and maintain continuous flow. A board is structured into defined stages—such as “To Do,” “In Progress,” and “Done”—allowing teams to track task progression at a glance. Each card represents a specific work item and includes key information like ownership, deadlines, and status. Work-in-progress (WIP). As a pull-based system, Kanban advances work only when downstream capacity is available, reducing overproduction and idle time. This approach supports consistent throughput, faster delivery, and a culture of accountability and continuous improvement.

An out-of-control process in Lean Six Sigma can have serious financial consequences. Defects and errors drive up costs through rework and scrap, consuming valuable labor, materials, and time. Poor quality or delays can lead to lost revenue as customers turn elsewhere, and in regulated industries, process failures may result in costly penalties. Operational inefficiencies—such as bottlenecks, overproduction, or underutilized resources—further erode margins and strain capacity.

From the customer’s perspective, these issues translate into reduced satisfaction. Quality also suffers when processes are out of control. Defect rates rise, and the loss of standardization disrupts the predictability and repeatability that Lean Six Sigma strives to maintain. An Out of Control Action Plan (OCAP) is a structured response protocol activated when a process exhibits signs of instability or special cause variation. It begins with immediate containment actions to halt or isolate the affected process, preventing further defects or downstream impact. RHLM conducts a root cause analysis using tools such as the 5 Whys, fishbone diagrams, or Pareto charts to identify the underlying issue. Based on these insights, corrective actions are implemented to eliminate the special cause and restore process stability. The final step involves verification and ongoing monitoring to ensure the process remains in control. OCAPs are typically standardized and embedded within the organization’s quality management system, enabling a rapid, consistent, and disciplined response to process deviations.

In Lean Six Sigma, an in-control process is one that performs consistently within its expected range, exhibiting only common cause variation—the routine, predictable fluctuations built into the system. This type of stability is verified using control charts, which display data points that remain within established upper and lower control limits and show no signs of unusual trends, shifts, or patterns. Such statistical control indicates that the process is stable and reliable, providing a solid foundation for improvement efforts. RHLM addresses process inconsistencies head-on—restoring control, driving stability, and laying the groundwork for continuous improvement.
